GALLIEN COUNSELING Thank you for selecting Gallien Counseling. We would like to provide you the most professional and effective services for promoting your well being. Our orientation and procedures in counseling are generally within a cognitive behavioral framework. Counselor Credentials: Angela Gallien is a Licensed Professional Counselor (LPC) in the state of Alabama. I also hold a National Counseling Certification (NCC). Confidentiality: All counseling activities, records, and identification information will remain confidential. The limits to confidentiality apply when the counselor considers a client to be in danger of harming others or him/herself, if the records are subpoenaed for some reason, or when the client grants disclosure of information through a signed release form. Alabama law requires that child abuse be reported to the Department to Human Services. Support staff will have access to information regarding clients only as they make appointments and handle day to day operations. Risk: Counseling often involves change. Processing areas of your life and learning new ways of thinking, feeling, and behaving can cause discomfort for you and those around you. However, if you are committed to your counseling process, you can expect benefits from our time together. Feel free to ask for a clarification that may help you feel more comfortable. Length of Session: The length of the session will be fifty (50) minutes. After Hours Contact: Telephone contact between the counselor and client is discouraged and will be limited to five (5) minutes. In case of an emergency, please call the on call number (365-7699) or the Crisis Center line at 323-7777. Telephone sessions lasting over ten (10) minutes will be charged at a rate of two (2) times the normal hourly fee. Referrals: The counselor reserves the right to terminate the counseling relationship for any reason deemed to be in the client’s best interest. If the counselor or client believes continued counseling is needed, the counselor will provide a referral. Support and Growth Groups: In addition to individual, marriage, and family counseling, Gallien Counseling offers small group therapy. Fees; The following schedule will be followed in computing
client’s fees: A BlueCross/BlueShield of Alabama provider. Please Note: Payment is expected at the time of service Cancellations: The client must cancel appointments 24 hours in advance of the scheduled session. Gallien Counseling does not practice double booking; therefore, the hour of your appointment is committed to you. The client will be charged the normal fee if the cancellation is not made 24 hours in advance.
